• Request a CellCheck Dry Cow consult by my nominated PVP (CCK TASAH-trained), delivered through the Targeted Advisory Service on Animal Health and funded through the Rural Development Programme (2014-2020).
• Consent to the processing of personal data as provided for herein.
• Consent to ICBF informing Animal Health Ireland of my eligibility against the above criteria and confirm that if considered eligible for a Dry Cow Consult I will:
• Engage with my nominated veterinary practitioner to develop and implement a selective dry cow strategy appropriate for my herd.
• Commence milk recording within the 1^st 2 months of the 2021 lactation.
• Following the consult, record all clinical mastitis cases thereafter on ICBF, by SMS or the Animal Events page.
• Record the date of drying off and product used for all cows at the end of the 2020 lactation.
  
  
• Consent and agree to ICBF (i) obtaining any data relating to animals which I either own now, have previously owned, or acquire in the future that ICBF shall consider relevant (including inter alia information relating to an animal's performance, health, and genetic makeup and related identifying personal data including herd owner name, address and herd number) ("Animal Data") from any source available to it including vets, Milk Recording Organisations, farm software providers and milk processors; and (ii) entering such Animal Data on its Database and using such Animal Data in any way it deems appropriate in providing its services and carrying out its activities including by making such Animal Data publicly available or passing it to third parties including those organisations listed below.

• Acknowledge that my personal data as provided in this form (i.e. my name, address, phone number and email address) ("Personal Data") will be retained by ICBF in conjunction with the Animal Data and may be used by ICBF when Animal Data is being compiled or assessed and also for the purpose of communicating with me.  ICBF's lawful basis for doing this is: the performance of a contract, its legitimate interest in administering its business, compliance with its legal and regulatory obligations and/or your consent (where this has been provided).  I also specifically consent to my Personal Data being given to the organisations for the purposes listed below and understand that I can withdraw this consent in respect of my Personal Data at any time by emailing ICBF at dpo@icbf.com.

DISCLAIMERS
By requesting a Dry Cow Consult and in consideration of my acceptance for registration, I acknowledge, accept, agree and confirm as follows:-

1. To the extent permitted by law that neither AHI nor the Database Operator nor any stakeholder, collaborator, officer, agent, subcontractor or employee of AHI or any member of the CellCheck TWG (all the foregoing, save AHI are hereinafter referred to as "Related Persons" or each individually as a "Related Person") shall have any liability or responsibility in respect of any error or omission in connection with the use, processing or publication of any data or otherwise in connection with the Dry Cow Consult or any loss or damage thereby incurred by any person provided that upon being made aware in writing of any error AHI shall procure the making good of such error as soon as practically possible and insofar as is within its powers.
2. That without prejudice to or limitation of the disclaimers herein contained or my acknowledgements or agreements or confirmations herein provided for in no event will AHI or the CellCheck TWG or any such person as referred to in the first paragraph of this clause have any liability to me for indirect or consequential damages or for damages for loss of profits arising out of or in connection with the Dry Cow Consult or the implementation thereof whether in relation to information provided or otherwise whatsoever.
3. That neither AHI, the CellCheck TWG, nor the Database Operator shall have any liability or responsibility in connection with any of the acts or omissions of any veterinary practitioner whether in connection with the Dry Cow Consult or services carried out in connection therewith or otherwise.
4. None of the above limitations shall apply in respect of any matter or claim for loss or damage to the extent (if any) such limitation is prohibited by law or contrary to public policy or which may restrict any mandatory, statutory obligation not lawfully capable of limitation.
5. The above terms and provisions and disclaimers shall be for the benefit of each of AHI, the Related Persons and each or one or more of them jointly as well as severally to the extent that they are relevant and capable of being of benefit to them as if each of them were party hereto.
6. Insofar as any provision hereof (including inter alia any disclaimer) is expressed to be or may be reasonably interpreted as being inter-alia for the benefit of (whether solely or with others) AHI, or any Related Person, AHI shall be deemed (without prejudice and without limiting in any way the rights and entitlements of AHI under this instrument and the benefit accruing to AHI pursuant to the terms hereof) agent of each such Related Person to the extent necessary for those persons to receive the benefit of any such provision unless AHI shall expressly disclaim or terminate any such agency by notice or in writing to me provided for the avoidance of doubt that AHI shall not be deemed by virtue of this provision to have any duties whether under the law of agency or otherwise in respect of any such person and any such duty or obligation is hereby denied and disclaimed.

In the event that I at any time have a complaint against any such persons in relation to the Dry Cow Consult (and including inter alia these provisions) or any matter arising in relation thereto I agree in the first instance to refer the complaint and dispute for investigation to the Chairman for the time being of the CellCheck TWG and shall provide all such information as the Chairman may require to investigate and if he shall deem fit to make a recommendation (by way of non-binding recommendation) for the resolution of the complaint or dispute.

I understand that any consent to use of my personal data given pursuant to data protection legislation may be withdrawn by me at any time.
